I looked up Julian James book at google books:

and then searched for;
The Origin of Consciousness in the Breakdown of the Bicameral Mind. ( You have to type the entire title in your search ).

There is the index, the chapters, readings from the chapters, etc. It does look like an interesting book, thanks for the tip.
