from college physics, the voltage is the energy derived from the existence of an electric force. This force is the product of electric charge and the electric field.
In order to get a very high voltage, we need to get a very high potential difference between the positive and the negative charges similar to the separation of capacitor's plates. And also the uniform electric field must be very strong.

This is just a perspective from the classical physics point-of-view. The quantum and relativistic points-of-view can become very complicated.
